Please provide the following information:
- Your name.
 - Your organization.
 - The recorder serial number (from the silver sticker on the back, with a bar code).
 - A description of what is happening, including any error code shown.
 - Confirmation that you have attempted the recommended course of action, if any.
 - Your email address (if you are leaving a voicemail message).
 - Physical mailing address where we would ship the replacement - no PO Boxes.
 - Phone number assocaated with the shipping address.
 
2. If your device is eligible for replacement, we will email you a Warranty Replacement Form and instructions.
Follow the instructions to pack and send the device to LENA for replacement. LENA Support will keep you posted on the status of your replacement.
